Didn't know I was suppost to sorry.



I think that is only for adoptees who have got the dog neutered after it has left our care, e.g a puppy. Jo will correct me if i'm wrong.

Its a form we give out to someone adopting a dog (mainly puppy) that's not neutered. Once they get the puppy neutered at 6 months their vets have to sign them and they send them back to us. If foster carer has forgot to give them out Fran will send them a copy x

I think this is something Fran sends out when doing the follow ups?

Its good if fosterers can give adopters one of these forms when adoption forms are signed, but it doesn't matter too much if not as I do email one if needed when I've done the follow up call.
